| Cost of Service: | $32.00 per day for the Adult Day Program or Frail Elderly Centre. $20.00 per day for Transition Group. Subsidies available. |

| Population Served: | Adult Day Program serves clients with a diagnosis of Alzheimer or other dementia. Program is offered in a locked unit. Frail Elderly Centre serves clients who are chronically frail with complex medical problems. Transition Group serves clients with mild cognitive impairment who are not at risk of wandering. Elderly Persons Centre serves clients who are independent, offering social and recreational programming. |

| Additional Information: | Monday to Friday from 9:00 a.m. to 4:00 p.m. - extended hours are available. Cost of transportation - from $7.00 to $17.00 per day. Transportation limits: Yonge and Steeles, Yonge and St Clair, Islington and Steeles, Islington and Eglinton. Some exceptions can be made. Contact Villa Colombo for further details. |
